Confined Space Entry Permit
A confined space is defined as having all below characteristics:
- The space is large enough and shaped so a person can enter and work.
- The space has a limited or restricted means for entry and exit.
- The space is not designed for continuous person occupancy.
If all three characteristics are present follow the below instructions for obtaining a permit.
- Click on the Confined Space Entry Request Form.
- Complete the request form and click the Update Permit Request button. The form allows document upload if needed. Requestor should complete the request form no fewer than 7 days before work is scheduled to start.
- Lamar University EHS & Risk Management will receive the request form and initiate an inspection of the confined space (see Appendix A and Appendix B).
- The requestor will be notified of the inspection finding and presentd with the permit.
Hotwork Permit
This program establishes guidelines to guard against fires from heat-producing operations performed by using portable equipment away from the shop area. Employees and contractors must be aware of procedures to follow when welding, grinding, cutting, or conducting other hot work operations.
The instructions for obtaining a Hotwork permit follow.
- Click on the Hotwork Permit Request Form link.
- Complete the request form and click the Update Permit Request button. The form allows document upload if needed. Requestor should complete the request form no fewer than 7 days before work is scheduled to start.
- Lamar University EHS & Risk Management will receive the request form and contact the requester with any follow-up. Also, EHS & Risk Management will send training modules for hotwork participants, including firewatch, to complete before approving the permit.
- The requestor should not begin the hotwork until receiving the permit.
- EHS & Risk Management will inspect the hotwork operation to assure compliance.
Potluck Meal Permit
EHS & Risk Management has assessed the risks associated with Lamar University Departments and Organizations holding potluck meals and has determined that potluck meals will be allowed if published guidelines are followed. This permit process assures the organizer is aware of these guidelines.
The instructions for obtaining a Potluck Meal permit follow.
- Click on the Potluck Meal Permit Request Form.
- Complete the request form and click the Update Permit Request button. The form allows document upload if needed. Requestor should complete the request form no fewer than 7 days before the event is scheduled.
- Lamar University EHS & Risk Management will receive the request form and contact the requester with any follow-up.
- EHS & Risk Management might decide to inspect the potluck event to assure compliance.
Fireworks, Pyrotechnics, Flame Effects, and Laser Entertainment Permit
This permit and related procedures set forth the standards and responsibilities for the application and approval of the use of fireworks, pyrotechnics, flame effects, and laser displays at Lamar University. These events include, but are not limited to, athletic competitions, ceremonies, conferences, banquet, science demonstrations, parades, or theatrical/concert performances.
- Click on the Fireworks, Pyrotechnics, Flame Effects, and Laser Entertainment Request Form.
- Complete the request form and click the Update Permit Request button. The form allows document upload if needed. Requestor should complete the request form no fewer than 7 days before event is scheduled.
- Lamar University EHS & Risk Management will receive the request form and contact the requester with any follow-up.
- EHS & Risk Management will issue the permit before the event begins.
