Baccalaureate Degree Nursing Program Application Checklist

To ensure eligibility, please carefully read and follow all instructions in the application instructions.

• Keep a copy of the Application Instructions, including this Checklist, to help you remember deadlines and other requirements.
• Ensure you are currently enrolled at Lamar University or have applied to Lamar University. Declare University Studies Health Science, Pre-Nursing Concentration, as your major on your application to Lamar University.
• If applicable, submit official transcripts from all universities and colleges attended to the Lamar University Office of Admissions, P.O. Box 10009, Beaumont, TX 77710.
• Schedule an appointment with your assigned academic advisor, if not already
• Schedule and complete the Evolve Reach Powered by HESI® Admission Assessment (A2) exam no later than the application deadline. Ensure that all required components of the HESI® A2 are taken (see full application instructions). To be eligible for admission, you must score 79% or better on the Reading Comprehension You may retake the Reading Comprehension section or the entire exam to achieve a minimum score of 79% in Reading. You must submit one set of your self-identified best HESI^® A2 session scores for consideration (Reading retake and Critical Thinking may be submitted separately).
• Before beginning the Dishman School of Nursing online application, ensure you have unofficial transcripts for all university/college coursework in PDF format, all HESI^® A2 scores in PDF format (if available), and a copy of military identification or DD214 in PDF format (if applicable). Screenshots and photos of documents will not be accepted.
• Complete and submit the Dishman School of Nursing online application and pay application fee by the deadline.
• Monitor personal email for communications from the Texas Board of Nursing (TBON) and/or Identogo regarding completing your Criminal Background Check (CBC). TBON will provide instructions on how to schedule an appointment for fingerprinting (if required). Once the CBC process is completed for this application cycle, submit the blue postcard (Blue Card), outcome letter, or declaratory order letter you receive to the Dishman School of Nursing Information Center (MMW Rm 100). The Blue Card (or if applicable, the Declaratory Order outcome letter) is a mandatory requirement to obtain full acceptance into the nursing program. Failure to schedule and complete your fingerprint appointment in a timely manner may result in ineligibility for admission to the nursing program.
• For in-progress coursework: submit official transcripts from all academic institutions attended to the Lamar University Office of Admissions after grades have been posted. Ensure Lamar University has received all official transcripts from other colleges/universities attended and has evaluated all transferable coursework; monitor Degree Audit (located in LU Connect under Banner Self-Serv) for application of transfer Submit unofficial transcripts for in-progress courses to nursing@lamar.edu after grades are posted (must be PDF format, with the name of the student and institution clearly visible; screenshots or photos are not acceptable). All required pre-admission and pre-requisite requirements must be shown as completed in Degree Audit before starting the nursing program.

IMPORTANT 

It is the responsibility of all applicants to the BSN nursing program to:

• provide and maintain up-to-date contact information to the Dishman School of Nursing Information Center, including phone, email, and address changes.
• be available: important messages may be communicated to you via phone or email; ensure you have voice mailbox set up on your phone and check your email often.
• be aware that the Dishman School of Nursing and the Texas Board of Nursing may contact you by email regarding your application. Please add the domains @lamar.edu, @identogo.com and @bon.texas.gov to your safe senders list in your email app. Also, check your spam/junk email folder often so you do not miss important information.
