Room Reservation Request

Directions:
1. This form is to be completed by faculty, staff, and approved non-Lamar entities only. Students are not permitted to submit room requests.

2. Consult departmental calendar before filling out this form. If you are requesting a room that the Music Department does not manage (i.e. University Theatre, Dishman Lecture Hall, Off Campus, etc.), then you must make sure that space is available before filling out this form. All rooms managed by the department are visible on the departmental calendar.

3. Once you find a potential date, fill out the form below electronically.

  • If faculty are requesting a recital date that will include a Lamar staff accompanist, then it is expected that the requesting faculty member has confirmation from the accompanist that they are available for the requested date prior to filling out this form.

4. Fill out one form per request (except for weekly/recurring requests). Each individual request must have its own form submitted.

Approval and Confirmation:
This form will be sent to the department chair for approval. Once approved, an administrative assistant will enter the event into the calendar and then email the requester and chair with confirmation.
