Student Health Center Policy Agreement

Our Mission

The Student Health Center is committed to ensuring easy and equal access to services for all enrolled students, regardless of income level, ability to pay, age, color, gender, disability, religion, national origin, race, sexual orientation, gender identity, and/or veteran status. Student Health Center services are available to currently enrolled Lamar University and Lamar Institute of Technology students.

Medications

Medications dispensed from the Health Center may be in non-child proof containers. Keep all medication out of the reach of children.

Contact Information

Please provide a CURRENT cell phone number and mailing address each time you are seen at the Health Center. If we cannot reach you by phone we may attempt to reach you through your university e-mail address and/or your postal address with a non-detailed message asking you to contact us.

Fees

A $10.00 fee will be billed to your student account for each visit to the Health Center. Medications, lab tests, and procedures will incur additional charges which will be applied to your student account unless your health insurance pays. It is your responsibility to inquire about charges before accepting service.

Privacy Practices

Our privacy policy is posted in the Health Center waiting room. Privacy policy brochures are available upon request. Your signature acknowledges that you have read, understand, and accept the above policies. Your signature also acknowledges your understanding of the privacy practices of Lamar University Student Health Center, which explains when, where, and why confidential health information may be used and shared.

Eligibility

All students of Lamar University and Lamar Institute of Technology who are enrolled in the current semester are eligible for our services. A one-time Health Center fee for the Fall and the Spring semester are included with your tuition fees. Summer and Mini-session fees are not automatically assessed and are collected only if the student uses the Health Center during those times.

Appointments

Appointments are required for all medical and counseling services. Every effort is made to accommodate patients with same day appointments that fit student schedules. If this is not possible, then the first available appointment will be offered. Please be a few minutes early to your appointment to allow for check-in and updating information. Students who arrive late for their appointment will be rescheduled for another date and time. Students who do not show up for their scheduled appointment or who fail to cancel/reschedule appointments 24 hours prior to the appointment time will be charged a $25 “No Show” fee for counseling appointments and a $10 "No Show" fee for medical appointments. To make a medical or counseling appointment, call 409-880-8466 between the hours of 7:30 am and 5:00 pm Monday through Friday.

Financial Matters

The Student Health Center office visit charge is $10.00. Any medications, treatments, durable medical goods, and procedures are in addition to the office visit charge. The student's account is billed for the amount of the visit. The account is payable at the cashier's office or online.

Private Insurance

Beginning September 1, 2016 the Student Health Center will no longer file commercial health insurance claims except for Academic Health Plans, the student health plan offered to Lamar University and Lamar Institute of Technology students, and TriCare, a military insurance plan.

If you do not have either of these plans we can provide you with a statement that you can use to file an insurance claim. The statement is available on the Patient Portal. We recommend contacting your insurance company before your visit to make sure we are in network and to verify coverage. Doing so will prepare you for any out of pocket costs that may become your responsibility. Remember that your are 100 percent responsible for all charges incurred and any balance will be billed to your student account.

No Insurance

We see all eligible Lamar University and Lamar Institute of Technology students, regardless of insurance coverage. The office visit charge is $10.00 and labs, medications, procedures, and durable medical goods such as wraps and crutches are an additional charge. Charges are billed to your student account and are payable at your cashier office according to their due dates and rules.

Academic Health Plans

Student health insurance coverage is recommended for all students, and is required for all international students.

Academic Health Plans covers the cost of office visits and lab work at Lamar University Student Health Center. The plan does not cover the cost of medications or durable medical goods (such as crutches and wraps) prescribed by the providers at the Student Health Center. It is your responsibility to inquire about the cost of medications and goods. The portion not covered will be applied to your student account.

If you choose to fill a medical prescription at an outside pharmacy present your insurance card with your prescription. Co-pays vary for each tier of prescription medication at a retail pharmacy.

Specifics of the policy, including benefits and premiums, are reviewed annually by the director of the Student Health Center. Visit the Academic Health Plans web site for detailed information, applications, and online enrollment. Brochures and applications are also available at the Student Health Center.
