Student Waiver of Access to Recommendations

The purpose of recommendations submitted on your behalf is to assist the Texas Academy of Leadership in the Humanities in making a decision on your candidacy. If you are admitted and you choose to enroll, recommendations sometimes aid in making rooming assignments and in advising and counseling. Under the Provisions of the Family Education Rights and Privacy Act you have the right, if you enroll at the Texas Academy of Leadership in the Humanities, to review your educational records. The act further provides that you may waive your right to see recommendations for admission.


Please complete all information below. Indicate your decision regarding access to recommendations submitted on your behalf by checking either "YES. ACCESS WAIVED" or "NO. ACCESS NOT WAIVED" below and signing your name (use mouse or finger). Review your information for accuracy and SUBMIT FORM. You will receive a confirmation that we have received your form. After you have received confirmation from us, you should complete the Recommendation Request form that is available in our FORMS and LINKS section of our website www.lamar.edu/texasacademy. Fill out the Request form completely, indicate the same access to recommendation choice that you indicated on this form, print 2 copies, sign both, and give one to your English teacher and one to your Counselor or Principal to request recommendations. The recommender will go to the same FORMS and LINKS section of our website and select the appropriate online recommendation form that corresponds with your selection "ACCESS WAIVED" or ACCESS NOT WAIVED" and will submit the appropriate online recommendation form to us online.
